  3. Budget: Ask how much therapy cost you, if you miss a session, cancel what is that cost?

  4. List things you want your therapist to help you succeed in.

    Endless spiral of thoughts. thinking has purpose but over thinking is not helpful. Catastrophize. Analyzisis paralysis. Relive mistakes from the past.

  • Text to friend. They do not respond right away. Thoughts “did I say something wrong? Are they mad at me? Are they ghosting me?

  • Behavior keep checking the phone, obsessively. body tense.

  • Consequences: Brain is wired to seek solution.

  • Back to Trigger. No resolution keeps the loop going. Keep checking phone all night long.

  • primary emotions, also known as basic or fundamental emotions, are considered to be happiness, sadness, fear, anger, disgust, and surpriseguilt, shame, envy, pride, and embarrassment

    5 reasons for this loop

  • 1. Anxiety: Amy seek to detect danger prep for danger. Anxiety sees every thing in alert mode. Snowball bad thoughts

  • 2 Perfectionism. Fear of getting it wrong, not good enough, fear of making mistakes or failure. this prevents action

  • 3 Fear of Failure. The what if Spiral. Anticipatory anxiety. the brain creates worry or fear over events or situations that might happen in the future. Problems might never exist. Avoid risk so miss opportunities.

  • 4 Past experiences: hippocampus links to past negative events. So a bad break up is life long warning sign for all relationships. Or social rejection is a survival threat rather than a lesson to be learned. Our past is not a puzzle to be solved but a series of lessons to be learned.

  • 5 Over Active Threat detection system DMN =Default Mode Network. Is active when not focused on a task. Spirals in worry. In worrier. If I think hard enough I will find a solution

  • SOLUTIONS to over thinking. 1. Name it to tame it. I am stuck in a worry loop. 2. Set a worry window. Set a timer then move on. Use distraction to move on. 3. Take action. Just choose one. Go for it. Analysis paralysis our brain needs data to choose what is safe. If this is you choose the smallest step forward. Over thinking is not who you are. It is just a habit you have developed.

  • Jared Sandberg Wall Street Journal 4-17-2007. “Strategic Incompotence.” that this takes practice. This is conscious and intentional actions, to under perform say “botch a spread sheet or burn dinner” so they are not enlisted to perform a task. This transfers the burden to someone else.

  • Media example Riding in Cars with Boys. “women want to forgive even a total screw up they want to help. It is in their nature or something. …you gotta tell them you need their help or else they won’t do it.” Or The sitcom The OFFICE . “People have to clean up after themselves”…. the staff confronts the temp worker that he needs to clean up the microwave after himself. His response is he will make it worse with use of a paper towel…. that he is hopelessness. And he walks off. " What would you do? Email HR tell them the temp is failing to follow health and safety rules? A colleagues direct line mgr?

  • It is not laziness or lack of expertise. It is about delegation. They struggle with sending basic invites, or only wait on tables without the restocking functions that all are expected as a function of closing. Do it quickly and undermine the task’s importance. It is not about being bad at it but avoid responsibility with the goal to shift power and avoid discomfort, or fear or failure or perfectionism. It creates a power balance issue one person does all the work leaving them feeling underappreciated and over worked, trapped. This thus erodes trust and mutual respect.

  • HOME FRONT: Messing up the grocery list task. “Can you wash the dishes so I can dry them? Sure what dishes. The dirty ones.” “how many? All of them. “ She notes the subject chucks the plate and throws a tantrum. Dry the bottles. It creates resentment as one person may feel it is easier to do it vs the work to get them to do it or "I forgot”. “you are just so much better at organizing…” It is literally one step. They act like you are unreasonable to ask or if the item moved location in the past decade. The baked in roles that some families get use to and thus do not expect a person to show up and do their task, role, part in life in that family, job, society.

  • Checklist. 1. Do simple tasks poorly. 2. Don’t take responsibility for their portion of work even when asked. 3. Others do their portion of work. 4. You do not trust then to do something when asked. 5 You feel taken advantage of. You know it is intenstional if they can do other things other areas.

    DO: 1. Bring it to their attention. 2. Let’s practice … together” present in non-judgmental way. They many not realize they are doing it. 3 set boundaries. Express your limits. 4 Let go of perfectionism. They will not do things your way. Do not rescue them to “do it right.” Notice your role in this. See video hyper independence. Do we not express our discontent. You do not have to do it ALL YOURSELF.

  • A potential protective effect of smoking and of nicotine on SARS-CoV-2 infection has been noted. Until recently [39], no firm conclusions could be drawn from studies evaluating the rates of current smokers in Covid-19. All these studies [40, 41, 42, 43, 44, 45, 46, 47, 48], although reporting low rates of current smokers, ranging from 1.4 % to 12.5 %, did not take into account the main potential confounders of smoking including age and sex. In the study that two of us are reporting [1], the rates of current smoking remain below 5 % even when main confounders for tobacco consumption, i.e. age and sex, in- or outpatient status, were considered. Compared to the French general population, the Covid-19 population exhibited a significantly weaker current daily smoker rate by 80.3 % for outpatients and by 75.4 % for inpatients. Thus, current smoking status appears to be a protective factor against the infection by SARS-CoV-2. Although the chemistry of tobacco smoke is complex, these data are consistent with the hypothesis that its protective role takes place through direct action on various types of nAChRs expressed in neurons, immune cells (including macrophages), cardiac tissue, lungs, and blood vessels.

    Mechanisms engaged in Covid-19 as nAChR disease might be tentatively suggested. There is structural evidence supporting the hypothesis that SARS-CoV-2 virus is a nicotinic agent. The recently reported X-ray structure of the RABV glycoprotein (G) ectodomain [49] shows that the region corresponding to the neurotoxin-like peptide is exposed at the G surface, in agreement with the fact that this region is part of the major antigenic region II of RABV [50]. The recently published cryo-EM structure of the trimeric SARS-CoV-2 spike (S) protein [51, 52] revealed an insertion with respect to that of SARS-CoV-1, in a loop that is disordered in the reported structure, and which has a polybasic sequence that corresponds to a furin site. Importantly, this exposed loop of the SARS-CoV-2 S protein also contains a motif that is homologous to that of snake neurotoxins and to the RABV neurotoxin-like region (Figure 1). This observation supports the hypothesis that SARS-CoV-2 virus itself is a nAChR blocker.

    Nicotine may be suggested as a potential preventive agent against Covid-19 infection. Both the epidemiological/clinical evidence and the in silico findings may suggest that Covid-19 infection is a nAChR disease that could be prevented and may be controlled by nicotine. Nicotine would then sterically or allosterically compete with the SARS-CoV-2 binding to the nAChR. This legitimates the use of nicotine as a protective agent against SARS-CoV-2 infection and the subsequent deficits it causes in the CNS. Thus, in order to prevent the infection and the retro-propagation of the virus through the CNS, we plan a therapeutic assay against Covid-19 with nicotine (and other nicotinic agents) patches or other delivery methods (like sniffing/chewing) in hospitalized patients and in the general population.

    In conclusion, we propose, and try to justify, the hypothesis that nAChRs play a critical role in the pathophysiology of SARS-CoV-2 infection and as a consequence propose nicotine and nicotinic orthosteric and/or allosteric agents as a possible therapy for SARS-CoV-2 infection. Interestingly, ivermectin, which has been recently shown to inhibit the replication of SARS-CoV-2 in cells in vitro [53], is a positive allosteric modulator of α7 nAChR [54]. The nicotinic hypothesis might be further challenged by additional clinical studies and by experimental observations determining whether SARS-CoV-2 physically interacts with the nAChR in vitro, for instance by electrophysiological recordings, high resolution EM and by animal model studies. Further work should also specify the still enigmatic relationships between ACE2 and nAChRs in the nervous system.

    One should not forget that nicotine is a drug of abuse [55] responsible for smoking addiction. Smoking has severe pathological consequences and remains a serious danger for health. Yet under controlled settings, nicotinic agents could provide an efficient treatment for an acute infection such as Covid-19. “
